  1. Within Acrobat, click on the Tools tab and select Prepare Form.
  2. Select a file or scan a document. Acrobat will automatically detect and create interactive form fields from the file you've uploaded. Most legible form fields can be detected and created automatically. If Acrobat cannot detect a particular form field automatically, you can quickly and easily add your own.

     
  3. Add new form fields from the top toolbar, and adjust the layout using tools in the right pane.
     
  4. Save your fillable PDF form and share it with others.
