I would only capitalize "form" if it is a proper noun. But if the sentence is something like "You will need to fill out the fourth and fifth form before..." etc.

Also, most citation styles recommend that with numbers under ten, you should write out the full number. So I would recommend writing out the word "fifth" rather than using "5th".
