To format the Header in Microsoft Word 2013 and 2016, double click in the location you want the header to be and click the Insert tab, and then choose header and footer, enter the information you want, then click Page numbers and then click the location you want the page numbers to be. You can double click into the header any time and change data. You will need to make sure the cursor is on the side you want to start typing, otherwise your page numbers will disappear.

To set your name, essay number and date as a Heading, simply put all of those items on separate lines (double-spaced), highlight them, and then click the "right justify" button on the toolbar. This button is located on the Home tab in the "paragraph" section. There are 4 alignment buttons and you can select the one that shows all the lines matched along the right side.
